I've faked a few pictures in my time and in my opinion this picture is a fake.
Having said that, many moons ago i downloaded a clip that showed exactly what this picture shows however it was a different girl and i can asure you the clip was real (I even showed it to my wife) She will never forget it!!!

Actually, this photo is not a fake in the normal way. If you see the clip at one of the unbirth-groups, you see that they have made a womans vagina out of latex and other skin-colored materials, and placed them on a sitting woman. She ask the man to head-fuck her, and he does. But you se from the rigid texture of her abdomen that it is just a fixed size prop. So, just some weird movie-magic.
