Well, Miss Panic, here is my attempt at an answer for that question. First of all it's one of every living thing's(land based at least) most primal fears; fire. Mesmerizing and terrifying at the same time. Then, the act of smoking itself is oral, with the mouth being opened for ease of use with the cigarette. Amazingly erotic, even more so with a woman's mouth because of the the transformations that lipstick can create. It's also a very visible sign of breathing, you can actually see the air that is exhaled as it comes from deep inside the object of desire. Then there is the smoke itself, atmospheric and beautiful and it has it's own hypnotic movement. It surrounds you. When I was a very small child I used to stare at the smoke curling up from a lit cigarette in my parent's ashtray as it snaked upwards into the air.

Plus, cigarette's used to symbolized power through their status as a desired commodity. They are still used as currency in prisons around the world and indicate social hierarchies. In the context of BDSM this becomes a very strong symbol indeed with its emphasis on authoritarian dynamics. So, there are a whole range of possible implications in the very simple act of smoking.
